EVIDENCE-BASED CARE AND REMOTE COLLABORATION

Abstract

The evidence-based practise (EBP) refers to a method which was devised for deciding to provide care in nursing, which has the basis on personal clinical expertise as well as recent research conducted based on the topic. The method applies the latest ways to provide high-quality nursing practices. These methods were fabricate based on research which was done to improve safety and health care of patients. Furthermore, the approach also integrates current technological advancement to patients in the healthcare system. For instance, remote supervision and diagnosis are becoming quite an essential method of delivering nursing care, especially to patients who are not able to access the healthcare facilities. The process also entails collaborating with other medical physicians and team member in person or remotely to reach a nursing standard which will ensure quality healthcare and safety for patients. It is, therefore, quite essential for nurses to learn this method for competency in the field and to ensure that proper care is accorded to patients. Understanding the interdisciplinary collaboration for nurses ensures that communication strategies are enhanced in the process of delivering care to patients (“Communication and Dissemination Strategies To Facilitate the Use of Health-Related Evidence,” n.d.). In this text, we will evaluate a case scenario where remote collaboration based on evidence care was applied. We will also assess evidence which was quite useful in the decision making the process and how EBP was used in developing the care plan for the patient.

Villa Health: Remote collaboration Evidence-Based care summary

The patient involved in this case scenario is a two-year-old kid named Caitlynn with both parents who are separated but not divorced. Caitlynn requires medical attention from a various medical professional for a condition which might proceed for the better part of her life. However, they reside quite far from healthcare, which could offer the required medical attention. The implication is there is a need for collaboration among parties involved such as the paediatrician, nurses, the respiratory therapist and parents. The doctors and pediatric nurse resolve to use remote treatment for the patient who also consists of the healthcare unit within the patient’s area code.

Analysis of relevance and effectiveness of decision making using evidence.

The action of decision making requires evaluation of various factors which may be complicated. While deciding on a pediatric nurse, it is essential to note that poor judgement of the situation could lead to undesirable outcomes. Therefore, it is quite necessary to make a neutral decision. In the case scenario, Caitlynn requires a proper evaluation of the since she needs care from a remote base as the condition may prolong, and she is quite young. The pediatric nurse, Virginia Anderson, plays quite a role in exploring the decision making process. For instance, she keeps all doctors concerned with the patient up to date by giving information about the patient. Virginia suggests since the patient requires a day-to-day treatment, then it is quite necessary to educate one of the parents although they are separated.

Virginia is also up to date with the practice-based nursing as she understands the importance of there being telemedicine relationship for the family in McHenry. She goes ahead and checks the presence of internet access in the residence of the patient by querying Madeline the social worker for the clinic at McHenry. in acquiring about resources in the house of the patient she was able to evaluate the kind of assistance to give the patient remotely. While the patient is at home, Virginia offers Caitlynn’s mother, Janice support with the chest physiotherapy. Rebecca, the physiotherapist, aids her. They examine the patient remotely with the assistance of a camera on skype. Virginia employed evidence-based factors to decide for the patient, which functioned quite well (Barret, 2017). Furthermore, Dr Copeland based his major conclusion from the diagnosis of cystic fibrosis which further aided in the decision-making process for the care of the patient.

Application of evidence-based practise model to address a practice issue

For a decision to be made about a patient, it is essential to know that evidence about the subject patient is quite vital. However, it’s also crucial to note that the evidence varies from country to another, just like the practice of medicine. Research by Consumer Reports of the National Research Center on 1,068 patients shows that what works for their condition is quite vital than their personal goals and values. Also, it was observed that patients prefer the trifecta elements consisting of clinician expertise, patient concerns and goals as well as medical evidence (“Evidence-Based Approach to Communicating Health Care Evidence to Patients,” n.d.).

In the research, it was also found that most patients agreed that the evidence-based medical decision was significant. It was also deduced that health provider should let patients know of their diagnosis and the risks involved in treatment regardless of whether the results are unpleasing or not.  It was concluded that patients who are listened by Medicare providers are more satisfied with the healthcare given to them.

In the case scenario, Janice is concerned about the marks from the percussion on Caitlynn after giving her the chest physiotherapy. After contacting Virginia, who responds together with Rebecca Helgo the respiratory physiotherapist, she is instructed to take the camera close to the marks. When the two view the dots, they realize that it was not entirely serious as it did not hurt Caitlynn. They advised her on the action to take while on the video call and suggested if the condition persisted the next step that could make such as acquiring the vibrating vest until the kid is old enough to perform the huff coughs. The use of technology for administering medical care is quite evident in acquiring evidence.

Plan care based on the available evidence

Evidence-based healthcare is recommended for several medical conditions such as diabetes, cystic fibrosis, and heart failure. in the past, patient safety research focused mostly on data analysis in the identification of issues related to patient safety. EBP requires evidence from scientific methods information on case reports and expert opinion (Nibbelink, & Brewer, 2018). For instance, before plan care for Caitlynn was made, there was a diagnosis for cystic fibrosis recommended by Dr Copeland. The diagnosis was conducted to attain evidence for the condition even though the patient had other conditions. Furthermore, academic education on its own is not enough in practising change. For this reason, the pediatric nurse provided a platform which enabled Janice to reach out to the healthcare practitioners.

In improving the safety and outcomes of a patient, it’s vital to consider engaging healthcare personnel who prioritize patient safety initiatives. The staff also advocate for clear communication of evidence base for patient safety topics and the setting to which it applies. For instance, in the case scenario, Virginia engages Marta Simmons in a conversation concerning Caitlynn and the process, she acquires information about the patient’s area of residence. The information she obtains aid in planning a health care model to fit the patient. For instance, she learnt of accessibility of internet by Janice. Additionally, technology was well embraced in Villa healthcare.

Conclusion.

Nurses and other healthcare personnel have the duty to come up with solutions in cases of a dilemma related to the patient. For the case scenario, we have been offered quite an excellent example of Virginia, who offers Evidence-based decision. Even though the coordination of remote healthcare might be difficult, the collaboration of professionals plays an essential role in making it functional. We have seen that teleconsultation also plays a vital role in providing insight and delivering healthcare services. Besides, interdisciplinary collaboration is also essential as it aids in the creation of communication channels. It is upon the healthcare systems to put into place up to date technological devices to facilitate communication with patients remotely as they produce better outcomes.
