Jane smith is a retired 58-year-old retired school teacher who lives at home with her husband and 18 year old son. She has been feeling unwell over the past few days and has been sent to A&E by her GP. Over the past few months, her mobility is decreasing due to breathlessness on minimal exertion and her deteriorating condition. She feels isolated and not able to engage in her hobby of gardening as she used to. Jane currently smokes about 5 cigarettes per day and is struggling to quit.
