It is common and understandable to make mistakes in our daily life activities. Companies and even big businesses have, at one point, made mistakes that may have or may not have resulted in losses, and those mistakes taken as a regular thing. When it comes to the medical side, a small mistake may call for a closure of the facility. The media and people will criticize, condemn, and even taint the name of the medical facility. To avoid all this technique, have been identified to assure quality and minimize such mistakes from occurring. Healthcare failure and effects analysis are some of the techniques.
Healthcare failure mode and effect analysis is the process of identifying possible failure, and the possible cause of those failures before future services are offered. It also offers opportunities for improving the current services since patient safety is its primary goal (R. Sadariya and Sudhakar). However, there are six steps required to complete the healthcare analysis failure and effects analysis. First, a clear definition of the topic to be studied is chosen. Secondly, it is to assemble a multidisciplinary team with experts and an advisor. Third, is to graphically describe the process by numbering each step in the diagram.
The Forth stage is to conduct a hazard analysis where all possible failures and risks are identified. The sever verity of each of the risk s are determined, and their possible cause is also analyzed and transferred into the HFMEA form. The fifth is to determine the actions and outcome measures. An action plan for each failure and risk will be through the HFMEA action working sheet, and lastly, the final stage is to follow up on actions taken. Follow up ensures that the actions were implemented and the possibility of the hazard happening should be very low
