Establishing measures

Measurement is a crucial component in testing and implementing changes. The measures inform on whether the changes will result in an improvement. Types of measurements include balancing measures, process measures, and outcome measures. This project will utilize a Qualified Clinical Data Registry (QCDR) measures as a MIPS measure. QCDR is a Center for Medicaid and Medicare Services (CMS) and verified vendor, which aims at improving the quality of healthcare. QCDR is more applicable and essential to clinicians.

An effective plan that the project will use to address the educational gap is the Academic Research for Clinical Outcomes. Academic Research for Clinical Outcomes defines the educational gaps in care needed to adopt evidence-based medicine. The plan addresses key gaps in support evidence and patient care.

To figure out the gap between what the staff knows and what they should know, I would use the knowledge assessment approach. The knowledge assessment approach is a self-assessment that helps in the cognitive understanding of principles, rules, decision-making processes, and other events going on in an organization. With knowledge assessment, I will understand the gap between the known and the unknown to the staff. For outcome measures, I will promote treatment and prevention of chronic disease, for balancing measures I will work with communities to develop effective practices that will further prevent chronic diseases. Finally, for process measures, I will implement the treatment and prevention procedures for chronic conditions that will result in desired outcomes with more affordable care.

Selecting Change

All changes do not necessarily lead to improvement though all improvements need a change. To improve the result, I will choose to change the working environment to attract more opportunities.
