Step 1: Identify the model of nursing care that you observed. Be specific about what you observed, who was doing what, when, how, and what led you to identify the particular model.
In studying the Finkerlman works, one notices the use of a nursing health care model identified as an Inter-professional practice model. The model acknowledges the importance of all the health professions working together with all the medical health categories through cooperation, collaboration integration, and condition to offer quality and reliable care at all times, as indicated by the IOM quality improvement assessments (Finkelman, 2016). The model involves the head nurse with a tight schedule coordinating and organizing the floor workers as they work in partnership with case management models, the dietary and rehab health care facilities, and units. The group usually includes various nurses from different expertise, and occupations meant to meets the health care needs of the patient. The model ensures that the objectives and obligations of the patients are met by enabling departments and various professions in health to work together. The floor nurses meet to plan on their day and move to every department assisting patients in multiple needs and ensuring that the requirements for their discharge are met before letting them go (Finkelman, 2016). They obtain the information of the patient’s from doctors and other departments and also integrate the case management model in their execution of patient’s safety. The team nursing model operates similarly to the inter-professional practice model. In this model, different nurses become controlled by one of them who is superior to others in terms of job status and level. The head of the team manages the operations and duties of the others. In the setting, the professional nurse coordinates the auxiliary nurses. The model mostly becomes applicable in the delivery department, where even the traditional midwives offer services under the supervision of a qualified nurse who offers their services to a specific group of small patients. However, the model effectiveness depends on the team leader’s success in communicating and ensuring that the environment in which they work in has an atmosphere of cooperation from the other members. Also, the team leader has to allocate responsibilities to each team member while taking and handling the entire management role of the team and providing direct care to the patients. Communication is critical in this model since it allows for a strong directive by expressing the responsibilities and duties required of every member clearly and elaborate in an understanding manner.
Step 3: Review and summarize one scholarly resource (not including your text) related to a different nursing care model than you observed in the practice setting.
The transitional care model is a caring way of looking after and treating people with chronic ailments. The models tend to concentrate on caregivers’ services improvement, which improves the outcomes for both patients and their families. Through this model, nurses provide care at home to the patients. Therefore through this model, there is a continuation of health care services for chronic patients from the time of release from the hospital to the period they stay at home, reducing the risk of becoming hospitalized (Rezapour-Nasrabad, 2018). However, the model requires close working ties between all the health care providers with the patient and their family. The nurses must also collaborate with other health caregivers and staff of various departments. In a study taken by Dr. Naylor on elderly patients suffering from heart failure, the rate of readmission significantly dropped due to the use of the transitional health care model. In this model, the nurse is the coordinator of the services, which include the regular visits of the nurse and the phone calls to the patient’s after their discharge to check how they are recovering (Rezapour-Nasrabad, 2018). However, its success depends on the team efforts of the patient, health care providers, including the nurses and all other practitioners and people involved with the treatment of the patient to work carefully so as information sharing can become comfortable and make the model and treatment effective.
Step 4: Discuss the different nursing care models from step 3 and how it could be implemented to improve the quality of nursing care, safety and staff satisfaction. Be specific.
The model applicability to patients with chronic diseases has already shown considerable positive impacts from the numerous research due to the ability to reduce the readmission cases significantly and extend the lifespan of those suffering from chronic diseases. However, the process can become more productive with the establishment and maintenance of trust between the relationship of patients and family (Rezapour-Nasrabad, 2018). Nurses require to generate confidence in the chronic and their families through follow up and phone calls to the patient’s family to make them have faith and trust in the system. Through this, they open up any necessary information concerning the patient without withholding any information for better provision of health care services. Also, the method becomes more effective through the assessment of the patients. When the patient gets evaluated wrongly, the health caregivers provide inadequate support and ineffective caregiving services. The use of technology also helps in motivating the nurse in doing their work since it reduces the fatigue and tedious working process of recording information, making them provide netter services (Rezapour-Nasrabad, 2018). Having adequate staffing to carry out the home care services motivates the workers since they don’t get tired from overworking. Workers who overdo tend to offer inadequate services due to lack of concentration. Consequently, it is essential to provide healthy working conditions for the health care providers and also for the patients.
Step 5: Summarize this assignment and what you learned about the two nursing care models discussed.
After thoroughly going through nursing care models, I came to realize that each approach has a direct impact on both the healthcare institution and patient care. I understood that each method was formulated to achieve specific objectives. I came to realize that nowadays, most healthcare institutions are trying to merge the models to provide quality health care. Each model has unique characteristics and provides care to specific groups. However, the models show clear and relating goals of ensuring that, in the end, the patient receives quality and better results from the health care facilities. Also, in executing the models and provision of the services, it is crucial to ensure the satisfaction of the healthcare providers and the cooperation between the stakeholders involved in health care provision. Lack of communication between the stakeholders also significantly affects the goals and objectives of the healthcare models.
Also, the type of the model is determined by the level of close relationship and working structure of the nurses in facilitating health care. However, critical aspects of each model are nurses and their team with the patient, whereby their coordination is required. The nurse’s and patient’s information sharing also determines the success of each model.
