Team-based care is getting increased focus on health service research to note cost-effective models to improve blood pressure control. Team-based care involves more health care providers from different disciplines managing patients. Our hypertension initiative focuses on the community of Detroit, Michigan, and the surrounding cities near Detroit. These community events are aimed at helping in addressing health disparities at the street level throughout the covered areas (the community of Detroit, Michigan, and the surrounding cities near Detroit), combining neighborhood health data with demographic information like fresh food, pollutants, income, demographics, race, and others. For years, according to the Center for Disease Control and Prevention, Michigan has had some of the highest rates nationally for cardiovascular disease, taken to be among the most preventable disease via lifestyle changes and medication. However, it is crisscrossed by hiking and biking trails and parks and lakes. For every 100,000, 298 residents die of heart failure, stroke, and other cardiovascular diseases in Michigan. That country rate is just below 257 deaths for every 100,000. The Center for Disease Control and Prevention noted that it is only in Nevada, and most southern states like Oklahoma, Louisiana, Mississippi, and West Virginia have worse rates than Michigan. Hypertension initiative focusing on community events can lead to an increase in Michigan adults who have their high blood pressure under control. As we keep linking people to new PCP or other doctors, we try to promote awareness and increase control of high blood pressure among the community of Detroit, Michigan, and the surrounding cities near Detroit. I work for Wayne State School of Medicine, and we have a hypertension initiative where we go all over the community of Detroit, Michigan, and the surrounding cities near Detroit. We host hypertension events for the community. We have a referral process involving hypertension initiatives through Wayne State, where we link people to new PCP or other doctors. It is also our responsibility to link those who don’t have PCP to link them to care. We also have recruitment in the community where we call businesses such as barbershops, casinos, clinics, parks, health fairs, hospitals, etc. and ask them if they would like us to take blood pressure from their customers, workers while they are working and patients.
We focus on taking their blood pressure because not many people take blood pressure or even have blood pressure machines at their homes. In addition to tasking blood pressure of these people, we also take demographic information like age, the city they live in, zip code, all three blood pressure readings that we take from them, we also take down their medical history like if they ever had a heart attack or stroke in the past. We take their height and weight as well, and we do this all over the city to better understand the city we’re trying to help. If we take down data and know how many people have high blood pressure compared to those who don’t have in the community. Most of our patients are African Americans since 60% of Detroit residents who are African Americans have high blood pressure.
As we continued with the events, we realized that some patients have never taken their blood pressure or didn’t even know what normal blood pressure means and how high blood pressure affects the human body. Some in the community were very uneducated about the topic, while others were more educated. This goes n line with previous research concerning blood pressure awareness in Detroit. A study found that many patients did not know their blood pressure level, nor could they accurately classify their level as elevated or normal. Hypertension and blood pressure awareness and knowledge, and the likely effect of blood pressure education programs have become essential recently. Blood pressure awareness leads to a higher compliance level with follow-up visits and medication. Research has also shown that a lack of knowledge concerning blood pressure and hypertension is a predictor of poor blood pressure control. Schwalm, McCready, Lopez-Jaramillo, Yusoff, Attaran, Lamelas &Yusuf, (2019) pointed to the need to improve hypertension and blood pressure knowledge and awareness to increase, education compliance as well as blood pressure control.
