The case for chapters 7 & 12 reflects on some of the challenges faced by Margaret, a woman aged 63 years, after suffering from hemorrhage, hypertension, and GEERD. Margaret is in recovery and stays in a nursing facility for specialized care. Having been identified at risk for falls, she is at risk for falls thus remain oriented and alert. The woman needs moderate assistance and uses the rolling walker to commute. She is scheduled for prednisone, GERD alendronic acid, and metoprolol, among other medications hence taken to a skilled nursing and rehabilitation home. Margaret stays alone thus, she must independent before returning home.

A week after being admitted into the facility, she develops swelling and pain in the right leg. Besides, Margaret has a low-grade temperature. According to the examination on blood, it is pointed that she has an elevated white blood cell count thus forcing the physician attending her to call for an immediate venous Doppler ultrasound (Buchbinder et al., 2019). The ultrasound was meant to examine the condition of her right leg. The results from the medical examinations show that she was positive for acute Deep Vein Thrombosis (DVT). As a result, she is transferred back to the hospital for acute care.

During the transfer, it is evident that there are diverse errors that occur, leading to medication faults. Carol’s decision to hand over envelope consisting of Margaret’s records without double-checking led to the mistakes. On the other hand, the receiving unit dealing with acute care in the hospital does not recognize that the patient and the chart do not match. As such, medication errors are reflected as significant quality control problems in the case. Similarly, the case presents a clear depiction of negligence in the documentation and its fatality in the healthcare sector. It led to the administration of wrong medications to the patient, thus risking her to fatal health complications and even death.
