Care Practice Guidelines
This unit requires you to examine theory and then apply this theory to practice.
Topics covered include reflective practice, values and person centred principles, care planning theory and practice and the use of models, collaborative working and legislation in planning and delivering care.
Relate this unit to your practice placement area.
Outcome 1
Explain reflective practice and why it is important in care. You might want to include an example of a model e.g. the stages in Gibbs cycle. Remember that by being reflective, we are able to examine our own attitudes, beliefs and practices. This should enable us to deliver care which is free from prejudice and is accessible to all who need it.

What is person centred care?
Choose 3 of the above and explain what they are.
- “Examine an ethical conflict or dilemma which you have experienced from practice or personal experience (500 – 1000 words)”
One of the features of care is that you will inevitably come across situations that pose an ethical dilemma. Perhaps a service user wants to do something that is considered to be unsafe under health and safety regulations or is not able to be accommodated due to staffing issues. Describe a situation and tell us how the person’s human rights and right to personalised care have been compromised.
Outcome 2
Using a case study or an individual you are currently working with
- Explain each stage of the care planning process and how this is applied in your workplace using a person centred approach
- Assessment (model) What is a care assessment? Why do we do it? Tell us how assessments are made in your workplace.
- Planning What is the planning stage? Take into account the types of care being delivered. Which model will be applied to the actions you are planning? If you are setting goals are they SMART?
- Developing How do you monitor the care plan? When are changes made?
- Implementing and evaluating
- Evaluating How and when are care plans evaluated or reviewed?
You must include how you communicated with the individuals and others connected with their care, how you ensured you took into account cultural and language needs. (1000 words) The importance of collaborative practice and how we work in partnership with others when providing quality care.
Include legislation and policy which ensures and enables individual rights and responsibilities within the care planning process (200 words)
Produce an effective care plan (500 words) recording and reporting
Evaluate your role and the role of others during this process. (Team/ group work) (300 words)
Remember the exercises in class. We have discussed the importance of accurate and detailed care plans as well as the importance of being person centred, values based, realistic etc.
Outcome 3
Reflective account (750 -1000 words)
“Describe and evaluate methods of working with individuals and groups in your care settings.
You need to include
- Using models of group work describe how you worked with service users and team members individually and or in a group setting
- Describe a situation when you have acted as an advocate and used counselling skills when supporting an individual
- Reflect on behaviour”
For this outcome you will look at different models of care. These are Egan’s Skilled Helper model, Roger’s Person Centred Approach, Group dynamics and behavioural approaches.
Explain each of these approaches and how each one is applied in your workplace.
Remember you need to evaluate these so tell us the positives and negatives of each approach.
When you are describing a situation when you have acted as an advocate, think of a time that you have helped a service user to speak up. It may well be that it relates to the ethical dilemma you described in outcome 1.
When you are reflecting on behaviour, this can be covered when you are describing behavioural approaches. Make sure when you are describing this that you acknowledge the point of a behavioural approach is to change someone’s undesirable behaviour.
