Outpatient Example 1:

Patient is discharged from the hospital after massive bilateral pulmonary emboli.  He shows up to primary care clinic on Thursday and states he will run out of Pradaxa on Sunday and cannot afford to purchase any additional tablets.  The attending physician consults with the pharmacist about what to do for this patient and we are able to use Xarelto samples and get the patient signed up for free Xarelto Patient Assistance Program (PAP) program.

Outpatient Example 2:

An elderly patient arrives for a Medicare Part D selection visit and medication review. The pharmacist recognizes the patient is falling a lot and has very low blood pressure.  The patient is on multiple blood pressure medications.  The pharmacist phones the physician and recommends changes to the blood pressure medication regimen.

Outpatient Example 3:

A patient arrives at the clinic for a medication review and complains of acute knee pain and urinary tract infection (UTI) symptoms.  The pharmacist reviews the patient’s medications and obtains a blood glucose level which is 400 mg/dL.  The pharmacist alerts the physician and nurse about the patient’s condition.  The nurse assists the patient and obtains a urine sample.  The nurse confirms the patient has a UTI.  The physician examines the patient and then consults with the pharmacist for pain and antibiotic regimen for the patient.

Outpatient Example 4:

Patient shows up to primary care clinic after her husband leaves her and takes all of her money.  She has a list of 10 medications which she gives to the attending physician. The physician asks the pharmacist to join the appointment and investigate which medications have PAP programs and which medications are on $4 lists/free lists at local pharmacies.

Inpatient Example 1:

The Surgical Intensive Care Unit (SICU) fellow enters an order for a total parenteral nutrition (TPN) consult for an overweight 24 year old male who suffered a 32% total body surface area burn following a grill fire.  As a member of a multidisciplinary Nutrition Support team, the dietician and gastroenterologist contact the pharmacist to collaborate on TPN appropriateness, as well as nutritional and micronutrient goals for this patient.

Inpatient Example 2:

You are an infectious diseases (ID) pharmacy specialist.  One of your pharmacy colleagues contacts you to assist in determining a home-going intravenous antibiotic plan for a patient on their service with a multi-drug resistant organism.  The infectious diseases team, including your ID attending, ID fellow, and outpatient ID clinic nurse perform rounds on the patient.  You contact the microbiology lab to determine additional antimicrobial susceptibilities for this patient.  Based on this data, you are able to provide a final recommendation for outpatient antimicrobial therapy (OPAT).

Inpatient Example 3:

An elderly patient is admitted to the Medical Intensive Care Unit with a GI bleed secondary to warfarin use and supratherapeutic INR.  Due to hemodynamic instability, central IV access is placed and the patient is started on the following medications:  norepinephrine continuous infusion, pantoprazole continuous infusion, vancomycin IV Q12h, piperacillin/tazobactam IV Q6h, and 0.9% sodium chloride with 20 mEq/L of potassium chloride.  The bedside nurse calls you to determine if all of the medications are compatible through the same IV line.
