Social Determinants of Health (SDOH) are multiplex conditions that play a vital role in determining the health of a person, a family, and the community. They include nonphysical factors such as socioeconomic, cultural constructs, political, and location-based conditions. SDOH factors affecting individuals today include affordable healthcare, accessible education systems, well-designed neighborhoods, safe environmental conditions, and the availability of healthy foods.

Family health assessments conducted in the previous assignment revealed that the family of Mr. Wayne Santos was affected by some very serious SDOH, which in turn affected their family health status. The availability of healthy foods is the most occurring factor, and the family is Asian-American Muslims with poor eating habits. In as much as the family strives to eat healthily, members of the family occasionally opt to go out for chunk food. Due to these poor eating habits, Mr. Wayne Santos himself has developed weight problems, and his mother has diabetes.  There are high chances her mother also got diabetes because of eating junk foods.

Lack of proper exercise is also another factor. In as much as most of the family members are engaged in physical exercises, these exercises do not seem to be enough nor routinely. Mr. Wayne only goes for exercises to cut weight that might not reduce any time soon. Besides, apart from the manual work done by the two brothers, there is no other exercise they are engaged in to keep them fit. Soon they too will add weight to a point where they develop weight issues blood pressure like their farther.   The family should collectively participate in physical exercises such as running at least every morning to help them maintain a healthy family.

Affordable health care is also a problem they face; Mrs. Wayne is partially paying for her medical bills; for the chemotherapy sessions, Wayne’s and his aging mother are also on medication.

Socioeconomic factors also press Mr. Wayne’s family, but they are, however, supporting each other in paying bills; they have learned to utilize the limited resources they have.

Some of the factors affecting this family are mainly due to negligence and ignorance. The family hinted that they take health issues very seriously when they provided information about the irregular visits to the hospital for a checkup. Moreover, all ailing members of the family have sought medical attention and are on medication. However, their nutritional lifestyle is still in question.

Based on the information gathered through the family health assessment, recommend age-appropriate screening for each family member. Provide support and rationale for your suggestions.

Receiving preventive care is the most important measure Mr. Wayne’s family can think of the aging members of the family are already on medications, and some of the aliments they have are hereditary. The family should take control of the situation and embark on the most effective preventive care not only of the aging group but also for the entire family.

The following are some of the best age-appropriate screening for each of the family members

Mr. Wayne

Mr. Wayne should enroll for prostate screening. Male Persons past 50 years are recommended to go for this screening. The family medical assessment. Mr. Wayne should also go for diabetes screening since the mother has diabetes, which increases the likely hood of him reporting positive due to his eating lifestyle.

With his weight problems, he should regularly go for obesity screening, cholesterol screening, and blood pressure screening.

Mrs. Wayne

She is highly recommended to go for breast cancer screening since she is within the highly susceptible group breast cancer, and she already has cancer. There are risks that the cancerous tissue might spread to the breast.

It is also important for her to register for cholesterol screening. She is within not only the age limit but also in a family indicated that they take junk food like the rest of Americans.

Rick

When the family go for regular checkups, rick is always missing; it is therefore advised that he goes for the regular general screening and of course kidney dialysis screening as he is always on drugs

Chelsea

The youngest daughter always uses the restroom at night; she should also go for early checkups.

This model is based on the notion that the information you receive will eventually shape your attitude, behaviors, and belief. This model, in some cases, might be ineffective in that it does not incorporate factors such as social, cultural, economic, and political factors.

This model would best fit the family because the factor affecting the family are related to matters of lifestyle. The family has a habit of eating red meat and chunk fatty food, and Rick is working and is not financially contributing to the family; this will culture unhealthy relationships between family members. Rick is also involved in drug and substance abuse

Their son Rick should stop drug and substance abuse, Mr. Wayne himself should make adjustments in his eating habits exercise effectively, and the entire family should take meals with fewer fats. If these measures were to be effected, Rick would participate in family role responsibility, and the family would be healthier. Thus, the provision of information that would influence this family to enact these changes is of high importance.

The following steps and strategies are observed in the behavioral change model for family-centered health promotion.

Family members Focusing on health practitioner’s perception of health needs when laddering health or medical issues within the family.

Family members Sharing knowledge to family members to increase awareness of the factors to improve family health. The sharing of health knowledge to family members will influence their choices on health issues.

Family members Educating the other members about health issues. It will influence the family members into using a better approach to health-related problems.

Introducing health campaigns in the family

Using a transmission approach to communicate health issues to family members

Applying a moralistic tone to emphasize the need for change in health issues.

Family members Emphasizing on medical problems that have a negative impact on the health of the family.
